Four checks before an NSAID leaves the counter

Screen the risk profile

Cardiovascular history, renal impairment, and previous GI bleed or ulcer — asked before the recommendation, not after. Age above 65 lowers the threshold on all three.

Lowest dose, shortest course

For self-limiting acute pain, agree an explicit stop point with the patient and state the circumstances in which they should return rather than repeat.

Check for duplication

Combination cold and flu preparations, topical products and low-dose aspirin all stack. Ask the patient to name what is already in the cupboard at home.

Give the safety net aloud

Two sentences: what to watch for, and precisely who to contact. A spoken warning is retained considerably better than a printed one.
